The duties of the HPA II include, but are not limited to: Assists with implementing the County PHC Program, serving as liaison between Public Health, contracted agencies, and workers serving on PHC; Develops and manages program databases using Excel or other software; conducts data analysis; and evaluates programmatic efforts to ensure the quality and effectiveness of services; Reviews monthly reports of contracted agencies to evaluate County PHC Program plans and progress; Prepares written reports describing analysis of PHC Program data and progress toward achieving overall goals; Researches best practices, policy change, regulations, and technical research, and makes recommendations to management regarding processes; Facilitates collaborative efforts between stakeholders, and provides technical assistance, advocacy and coalition building for workers, CBOs, and employers; Coordinates and leads meetings between Public Health, contracted agencies and stakeholders to discuss and influence policy issues impacting the County PHC Program; Supports contracted agencies to develop and implement action plans, and works with agencies to ensure progress toward successful scope of work implementation; Prepares position papers, recommendations, and summary reports on issues relating to the County PHC Program; Prepares and/or analyzes grant applications and monitors County PHC Program related activities and services to ensure quality assurance with grant activities; Helps implement awareness campaigns that promote the County PHC Program; designs, lays out, produces, and disseminate products such as flyers, newsletters, pamphlets, and articles; Complies with all required Public Health trainings; and Carries out other projects, as assigned. Minimum Qualifications: A bachelor's degree from an accredited college or university and four years of relevant experience; and A valid California Class C Driver License or the ability to utilize an alternative method of transportation when needed to carry out job-related essential functions.
