Develops and implements transition plans, as indicated, to ensure continuity of care. Negotiates and documents single case agreements according to the company P&P's procedures. Reviews planned, in process, or completed health care services to ensure medical necessity and effectiveness according to evidence-based criteria. Proposes alternatives when the requested services do not meet medical necessity criteria or are outside the contracted network. As assigned and based on credentials, monitors and reviews specialized requests and treatment records such as Treatment Record Forms. This role is a long-term contract with our client. Due to COVID the initial period of the engagement will be remote work before moving to onsite when facilities reopen. Onsite work can be based out of Raleigh, NC or Eagan, MN. For candidates who possess 5-9 years of experience, this position will pay an hourly rate of $40-$45/hr. on a W2 basis w/ option of benefits, NO PTO/Holiday, or an annual salary of $75K-$85K on a W2 basis w/ options of Benefits 2 weeks PTO and 2 weeks Holiday, or an hourly rate $50-$55/hr. on a 10-99/CTC basis.
